Hair Restoration via Direct Hair Implantation (DHI Method)

Clinical Overview

Direct Hair Implantation (DHI) is an advanced, minimally invasive hair restoration technique that involves the extraction and immediate implantation of individual hair follicles using a specialised implanter. Unlike older methods, DHI does not require incisions or channel creation in the recipient area. This approach allows for precise control of angle, depth, and direction during implantation, ensuring a dense, natural-looking result with minimal trauma and no linear scarring.

Ideal Candidate Profile

  • Male or female patients experiencing patterned hair loss, diffuse thinning, or receding hairlines
  • Individuals with sufficient donor hair at the back or sides of the scalp
  • Candidates seeking natural results with minimal downtime and no surgical scars
  • Patients with realistic expectations and stable hair loss progression

Treatment Protocol

Comprehensive Assessment:

A detailed consultation is performed, including medical history, photographic documentation, and magnified trichoscopy to evaluate donor density and graft requirements.

Pre-Procedure Preparation:

On the day of treatment, the donor and recipient areas are trimmed and cleansed. Local anaesthetic is administered to ensure complete patient comfort.

Graft Extraction:

Using a motorised extraction device with a micro punch (typically 0.6–0.8 mm in diameter), follicular units are carefully harvested from the donor zone. These grafts are immediately stored in a physiological solution to maintain their vitality.

Implantation Using the DHI Technique:

Each graft is implanted directly into the recipient area using a precision implanter. This tool allows for meticulous placement of each follicle without the need to create incisions beforehand. Hair is implanted at a natural angle, depth, and direction, replicating the original growth pattern of the patient’s hair.

Completion and Aftercare Briefing:

Once implantation is complete, the scalp is gently cleansed. A tailored post-operative care pack is provided, and detailed instructions are reviewed to ensure optimal healing.

The procedure typically lasts 6 to 8 hours depending on the number of grafts and may be split over two sessions for larger cases.

Post-Treatment Care Guidelines

  • Sleep with your head elevated for 7–10 days to reduce swelling and protect the grafts
  • Avoid touching or washing the implanted area for the first 48 hours
  • Begin gentle cleansing from day 3, using the technique and products provided
  • Avoid physical exertion, sweating, direct sun exposure, and wearing hats or helmets for at least 10–14 days
  • Refrain from alcohol, caffeine, nicotine, and supplements that could increase bleeding or inflammation for one week
  • Use all prescribed medication as directed, including antibiotics and anti-inflammatory agents
  • Return for follow-up appointments to assess progress and receive guidance on hair washing and care

Recommended Treatment Frequency

  • Initial results become visible after 3 to 4 months as new hairs begin to grow
  • Full growth and final density are achieved between 9 and 12 months post-treatment
  • A second session may be considered after 12–18 months in cases requiring additional density or coverage
  • For optimal outcomes, supportive treatments such as Hair PRP may be introduced one month post-transplant to enhance follicular strength and improve long-term hair quality
